Based in Chesterfield Missouri, Delmar Gardens Home Care is a Medicare/Medicaid, state-licensed and certified agency offering skilled at-home, intermittent care for patients. Highly qualified nurses, rehab therapists, social workers and home health aides work in harmony to address each patient's individual needs. Our staff is caring and concerned, well-trained and highly qualified. Each member of our team is carefully screened and fully insured. Services are offered 24 hours a day, 7 days a week.

Care
Delmar Gardens Home Care is committed to providing professional care to homebound patients. Specific needs and unique factors which influence each patient's care are taken into consideration. A Registered Nurse serves as case manager and makes a complete assessment of the homebound patient. Working in conjunction with the attending physician, a plan of treatment is prepared for each patient.

Rehabilitation Services
Our staff of expert speech, physical and occupational therapists works together to restore patients to a healthy physical and psychological state.
Physical Therapy
Following orthopedic surgery, illness, onset of neurological conditions, generalized weakness and other conditions affecting mobility, the licensed physical therapist and team members develop a program to improve each patient's mobility and to achieve his or her maximum level of functioning.

Occupational Therapy
Loss of function in the arms, difficulty with dressing, bathing, meal preparation and other functional activities as well as adaptive equipment needs are all addressed by a registered occupational therapist. Our therapists and assistants also help patients with cardiopulmonary, neurological and orthopedic conditions, as well as those lacking endurance. Occupational therapy is meant to allow the patient to be as independent as possible.

- Neurological, orthopedic, and traumatic conditions that affect the ability to speak, swallow, problem solve, word find, and other related communication difficulties are addressed by a licensed speech pathologist. This specialized therapist assists the patient in achieving his or her maximum potential relating to communication and oral-motor skills.

Specialized Care
CHF (Congestive Heart Failure)
More than three million Americans have congestive heart failure, and as the population ages the incidence of congestive heart failure is rising rapidly. Our program helps to keep the patient out of the hospital and decrease rehospitalization.
Wound Care
Some wounds are deep and need medical attention to prevent infection and loss of function, due to damage to underlying structures like bone, muscle, tendon, arteries and nerves. The purpose of medical care for wounds is to prevent complications and preserve function.
Diabetic Care/Training
Diabetic patients have special dietary and exercise concerns that cause bigger problems if neglected. Education must be intertwined with medical treatment. Our team helps to develop and monitor a diabetic teaching plan.
COPD (Chronic Obstructive Pulmonary Disease)
Prevent further deterioration in lung function, alleviate symptoms and improve performance of daily activities and quality of life. Chronic obstruction of the flow of air through the airways and out of the lungs, generally is permanent and may be progressive over time. Delmar Gardens Home Care helps to develop and maintain a care plan.

I want you to know how much my home care girls helped me. I am so much improved. My balance is very good. My use of legs and arms are so much improved.
